These supports were woven into an Independent Living Plan outlining goals for healthcare access, social engagement, and vocational success. Mood improved as the consumer attended a wellness center, and a photo from their first day on the job—grinning ear to ear—showed us they were gaining control.
Over the past year, CIL assisted with an SSI application, and the consumer now receives that crucial benefit. They learned self-advocacy skills to secure consistent hours in a well-suited kitchen position. Stable housing, reliable transportation, meaningful work, and referrals to healthcare, food pantries, therapy, home repair programs, meal delivery, and legal aid have all contributed to their growing independence.
Looking ahead, we’ll revisit the DDD application documentation, connect them to more social opportunities, and help build natural supports so this consumer continues to thrive long after our direct involvement ends. Through every twist and turn, CIL remains their unwavering partner in independence.
